Normally P1491 (EGR valve insufficient lift detected) means the EGR passages are plugged with carbon. The passages are much more of a problem than the valve and all the system knows is that the flow - not the lift - is inadequate. However, insufficient flow will not cause your engine to run roughly, excessive flow will. and that will not bring in a check engine light. Putting the two of those together makes a good case for the EGR valve being carboned up, causing it to not close or open consistently. It can be cleaned or replaced. It is rare for EGR valve replacement to fix flow problems but with the rough running your odds are much better.
